The Case Management Director is responsible for the development and operations of the Case Management Department. The scope of responsibility includes utilization management, social work, and clinical documentation nurses. The director establishes the direction and activities of the department consistent with Huntsville Hospital’s strategic plan and mission.
The RN Case Manager is to support the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. This role integrates and coordinates Utilization management, care coordination and discharge planning functions. The Case Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of stay, and promote efficient use of resources.

Performs Radiology procedures in accordance with policies and standards in effect for the Radiology Department. Cooperates and works effectively with fellow Radiology employees and other members of the healthcare team. Exemplifies professionalism in working with all outside health agencies, physicians offices and outpatients. Prepares and maintains patient records.

The Patient Care Aide must exhibit the ability to work with people from diverse socio-economic backgrounds; to organize, set priorities and manage multiple priorities, and to adapt to a rapidly changing environment.

Responsible for assisting the physician in patient treatment issues, providing direct patient care, and maintaining proper standards of care. Performs patient check out procedures, schedules follow up and/or referral visits and collects deductibles and co-pays. Assists physicians and resident physicians when completing physical examinations. Obtains laboratory specimens and prepares them for transport. Provides clerical support and maintains systematic initiation and follow up of scheduled events.
Performs ultrasonography procedures at a technical level not requiring constant supervision. Uses independent judgment ingenuity and initiative to perform state-of-the art diagnostic ultrasound procedures.

Executes therapeutic physician orders and therapeutic nursing interventions. Evaluates the patient's response to care and the attainment of or progress towards patient outcomes. Coordinates care between other professional disciplines as needed. Helps patients and/or staff identify problems and explores with them the alternative choices and outcomes.
The Renal Specialist Level I will identify the needs and priorities of the dialysis and apheresis patient through assessment. The Renal Specialist Level I plans and delegates care based on patient needs. The Renal Specialist Level I evaluates the patient’s response to care and the attainment of or progress toward expected outcomes. The Renal Specialist Level I is accountable for outcomes of patient care of assigned patients
The Licensed Practical Nurse (LPN) performs a broad array of nursing tasks under the supervision of a registered nurse or provider. The individual provides direct patient care for diverse patient populations that vary in gender, age, socioeconomic status, education, physical and mental abilities, locations, sexual lifestyle, and racial and ethnic background. The LPN implements technical nursing procedures according to established policies and procedures and plan of care, assists and reinforces routine patient education, and documents patient care appropriately. The LPN communicates and interacts with other team members to facilitate teamwork and delivery of quality care.
