The registered nurse is responsible for the delivery of patient care using the nursing processes of assessing, planning, implementing and evaluating care. Also directs and guides patient care activities of ancillary personnel while maintaining standards of professional nursing. Cooperates and communicates well with all health team members to maintain the highest quality patient care.

Additionally, under the direct supervision of licensed nurses, the nursing assistant performs various duties to assist in the care of patients in accordance with established policies and procedures.
These duties include, but are not limited to, bathing, mouth care, ambulation assistance, toilet assistance, feeding, measuring vital signs and blood glucose, and transporting.
The RN identifies patient needs and priorities for care through assessment. The RN plans and delegates care based on patient needs. The RN evaluates the patient’s response to care and the attainment of or progress toward expected outcomes. The RN is accountable for outcomes of patient care delivered during the assigned shift.
The Licensed Practical Nurse (LPN) performs a broad array of nursing tasks under the supervision of a registered nurse or provider. The individual provides direct patient care for diverse patient populations that vary in gender, age, socioeconomic status, education, physical and mental abilities, locations, sexual lifestyle, and racial and ethnic background. The LPN implements technical nursing procedures according to established policies and procedures and plan of care, assists and reinforces routine patient education, and documents patient care appropriately. The LPN communicates and interacts with other team members to facilitate teamwork and delivery of quality care.
Communicates data, information, knowledge, and wisdom in nursing practice. Serves as a liaison between clinicians, ancillary departments, and Information Technology (IT) to
align clinical and IT standards with department workflows, processes, and policies. Acts as a change agent for hospital leaders and clinicians to promote adoption of new processes
and technologies. Serves as the clinical leader for projects involving technology changes that impact nursing and vice versa with the clinical team members. Ensures appropriate clinical leaders are included in decisions involving information technology. Coordinate clinical processes and workflows across the organization that involve both nursing and information technology.
Serves as a clinical informatics expert to directors/managers and staff. Evaluates and coordinates the education plan to ensure all clinical staff are trained. Ensures policies and procedures are updated that involve information technology.

Monitors drug therapy to improve patient outcome and quality of care, recommends rational and cost-effective pharmacotherapeutic alternatives and provides accurate and efficient medication distribution. Implements and evaluates nutritional support and pharmacokinetic regimens.
Supervises pharmacy technical staff.

Job Summary
Centered in a clinician provider practice, this individual acts as a collaborative member of the clinical team in providing continuity of and quality patient care. Initiates patient care assessments, gathers vital signs, prepares patient and exam room for procedures, and collects specimens. Coordinates care for patients with chronic disease and manages effective care transitions for them within the continuum. Partners with the provider care team for successful preventative care visits to reduce the severity of chronic disease and avoidable acute illness. Provides effective clinical health coaching to assist patients with self-management of their chronic disease and life-style changes to mitigate health risk.
