Promotes optimal patient outcomes by assisting with medication reconciliation. Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the hospital. Compiled history information is entered into the patient’s electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care. Seeks assistance/clarification from a pharmacist with any questions or issues that may arise.
The Clinical Charge Nurse identifies patient needs and priorities for care through assessment, planning and delegating care based on patient needs. Executes therapeutic physician orders and therapeutic nursing interventions. Evaluates the patient's response to care and the attainment of or progress towards patient outcomes.
