Job Summary    Performs physical therapy procedures and other related tasks under the direction of a Registered Physical Therapist (RPT) and assists the RPT in patient activities.  Supervises technicians, volunteers and students as assigned.

The Outpatient Charge Nurse identifies patient needs and priorities for care through assessment, planning and delegating care based on patient needs. Executes therapeutic physician orders and therapeutic nursing interventions. Evaluates the patients response to care and the attainment of or progress towards patient outcomes. The Licensed Practical Nurse (LPN) performs a broad array of nursing tasks under the supervision of a registered nurse or provider. The individual provides direct patient care for diverse patient populations that vary in gender, age, socioeconomic status, education, physical and mental abilities, locations, sexual lifestyle, and racial and ethnic background. The LPN implements technical nursing procedures according to established policies and procedures and plan of care, assists and reinforces routine patient education, and documents patient care appropriately.  The LPN communicates and interacts with other team members to facilitate teamwork and delivery of quality care.

Provides medical nutrition care to patients through nutrition assessment, diagnosis, intervention and education utilizing evidence based guidelines.  Develops care plans with appropriate follow-up and reassessment of intervention. Manages the total nutritional care of patients receiving Enteral Nutrition. Assess and diagnose patients for malnutrition.

This position is responsible for developing, implementing and managing the efficiency and effectiveness of the Patient Access Departments. This includes activities in scheduling, registration, and telephone room. This position will work closely with the Director of Operations and Patient Care Director to implement processes to improve patient access.
