The Registered Nurse (RN) organizes and coordinates delivery of nursing care to groups of patients in compliance with hospital and departmental policies, protocols and procedures.  He/she provides direct patient care by monitoring physiological and behavioral status, evaluating patient progress and the effectiveness of interventions, and by performing treatments and procedures.  He/she recognizes, reports and documents clinical observations.  The Registered Nurse participates in orientation, training and management of personnel.

Job Summary: The Care Transition Coordinator will work in a variety of outreach settings (including emergency room, outpatient departments and inpatient nursing units) to provide care management for frequent utilizers of DMH services with a variety of complex health care needs. Responsible for conducting Psychosocial Assessments, group therapies, and other classes, teach group skills to unit staff (as needed); incorporate knowledge in identifying patients’ needs in developing and implementing multi-disciplinary treatment plans. Performs routine and specialized moderate and high complexity laboratory tests on all types of blood and body fluid specimens under minimal supervision. Insures accurate and reliable test results by adhering to a strict quality control program. Performs instrument function checks and preventative maintenance procedures according to department’s policies and procedures. Maintains records and statistical data according to regulative requirements. Teaches new employees and students laboratory policies and procedures. Assists as needed in all laboratory areas as required to maintain appropriate patient care.

The Speech Therapist assumes primary responsibility for evaluation of the patient, planning, implementing, supervising, reevaluating and changing the treatment program.
