Overview

Job Summary :

The ambulatory pharmacist works to provide comprehensive pharmaceutical services within Huntsville Hospital clinics. Pharmacists will meet with eligible patients during in-clinic appointments to provide education about closed-loop care. The pharmacist is responsible for ensuring enrolled patients are successfully treating their specialty condition to ensure the patients lives are improved as measured by specific, objective metrics. The pharmacist partners closely with the ambulatory pharmacy technicians to understand patient issues, such as insurance claim results, general counseling needs, and adherence concerns, and assists in resolutions as needed.

Responsibilities

Key Responsibilities and Essential Functions:

Collaborates with providers and clinic staff as an embedded member of the clinical delivery team in the outpatient clinic where assigned.  Serves as a subject matter expert on the key specialty medications that are prescribed in the assigned clinic, and engages with patients during their visit to provide medication education, injection training, and other medication related activities as needed.  Assists with recruitment and enrollment of patients into specialty pharmacy services provided by Huntsville Hospital to ensure the best possible clinical outcomes.  Proactively reviews clinic schedules for potential specialty pharmacy candidates , effectively prioritizes scheduling of face-to-face clinic interactions, and supports patients for the duration of their therapy.  Engages in payer-related activities such as benefit investigations and prior authorizations.  Identifies and resolves medication-related issues and provides drug information to patients, families, physicians, nurses, and other healthcare professionals.  Coordinates clinical projects and assists with formulary decisions, as needed.  Ensures billing compliance and revenue capture for assigned clinic.  Serving as a resource and drug expert in specialty medications and increasing knowledge through research and writing activities.  Adequately evaluates and monitors patient drug profiles for potential drug-drug interactions, allergy data, height, weight, and pregnancy/lactation status as indicated.  Addresses inquiries from clinic staff/providers and patients/families.  Documents clinical activities and prevented medication errors.  Oversees daily operations of patient care coordinators and assists with interviews, training, evaluating, and disciplinary actions.  Provides feedback to pharmacy management regarding operations, staffing, training, and departmental procedures.  Incorporates medication safety standards and accreditation standards into daily practice.

 

 

Qualifications

Minimum Knowledge, Skills and Experience required:

 

Education: 

Doctorate in Pharmacy (Pharm.D.) required

 

License, Certification and/or Registration:

Must be registered with the Alabama State Board of Pharmacy as a Pharmacist, and registration must be in good standing.

Proof of current AHA BLS must be provided or obtained by the end of the orientation period.

 

Experience:

3-5 years of experience required

 

Additional Skills/Abilities:

Proficiency in Microsoft Office including Word, Excel, PowerPoint and Outlook Calendar.  The ability to self-start a program, execute quickly and accurately without pre-defined processes and protocols, and an interest in defining such processes and protocols for the future.  Strong people skills and the ability to work with teams across various clinics and health systems partners.  The ability to communicate effectively at all levels inside and outside the organization.
