The Care Coordination Liaison serves as a key point of contact between patients, healthcare providers, local clients’ employees, and internal teams to ensure smooth navigation of primary care, specialty services, lab work, imaging, and procedures within the HH Health System. This role manages inbound communications through calls, emails, and faxes while collaborating with the Corporate Care Team to maintain efficient workflows. The Liaison is required to work onsite at HH Urgent Care in Madison on designated days to coordinate referrals directly with physicians and staff, utilize the EMR system, and ensure timely processing of patient needs. Building relationships with providers and staff is essential to facilitate appointments and provide a positive patient experience. Responsibilities include data collection, reporting, accurate documentation, and addressing care coordination gaps. By managing referrals effectively and supporting patient retention efforts, the Liaison helps ensure continuity of care. This position reports to the Senior Manager of Corporate Health Business Development.
Education required: High school diploma/GED
Education preferred: Associates or Bachelor's degree
Experience: Prior customer service, health insurance, care management, scheduling, and/or related experience

· Knowledge of health care environment. |
· Computer literate with the ability to learn new software applications, develop reports, and review data. |
· Excellent communication and customer service skills required. |
