Overview

The Coordination Specialist serves as the key intermediary between patients, healthcare providers, local employers’ employees, and internal teams, ensuring seamless navigation or primary care, specialists, lab work, imaging, and procedures within the HH Health System. This role manages inbound communications including calls, emails and faxes, and coordinates with the Corporate Care Team to streamline workflows and optimize efficiency. The specialist builds relationships with provider scheduling teams to secure timely appointments and ensure patients receive necessary care without delays. Responsibilities include data collection, reporting, and accurate documentation to facilitate smooth handoffs. Additionally, this role identifies and resolves care coordination gaps to enhance the patient experience. By effectively managing referrals and optimizing patient retention. Specialist must be flexible and may be required to work on site at HH Urgent Care as part of the position coverage requirements. The position reports to the Senior Manager of Corporate Health Business Development.

Qualifications

Education required: High school diploma or GED

Education preferred: Associates or Bachelor Degree.                               

Experience: Prior customer service, health services, health insurance, case management, scheduling, and/or related experience. 

Additional Skills/Abilities: 

  • Knowledge of health care environment.
  • Computer literate with the ability to learn new software applications, develop reports, and review data.
  • Excellent communication and customer service skills required. 
  • Attention to details and ability to handle multiple tasks simultaneously. 
  • Ability to problem solve and communicate effectively.
