The RN Mental Health Navigator identifies Behavioral Health admission assessments and any required nursing assessments with documentation for Behavioral Health patients in the Emergency Department through assessment, planning and delegating care based on patient needs. Evaluates the patient’s response to care and the attainment of or progress towards patient outcomes. Coordinates care between other professional disciplines as needed. Case management for Behavioral Health patients holding in the Emergency Department to facilitate the transfer process to other facilities. Helps patients and/or staff identify problems and explores with them the alternative choices and outcomes. Admission assessments on Behavioral Health unit as needed.
Admission assessments for BH patients in the Emergency Department per HH policy
Documentation of required nursing assessments on BH patients in the Emergency Department
Facilitate the transfer process for BH patients holding in the ED per provider orders
Ability to establish collaborative practice with other healthcare professionals regarding plan of care, variances in plan implementation and patient outcomes.
Facilitates discharges on BH patients in the Emergency Department with various community providers and facilities and setting up outpatient follow up appointments
Communicates effectively with Unit Directors, supervisors, patients, guests, physicians, and co-workers
Other duties as assigned.
Education:
High School Diploma/GED required.
Graduate of an accredited school of nursing required.
Current Alabama Nursing License required.
BLS certification is required within 90 days of hire.
