Key Responsibilities / Essential Functions
Patient Care Coordination
• Serve as the primary point of contact for patients receiving dialysis treatment.
• Coordinate the scheduling of patient’s dialysis treatments.
• Monitor patients’ progress, lab results, and care plans in collaboration with nephrologists and nursing staff.
• Facilitate access placement referrals and follow-up.
Clinical Oversight & Support
• Ensure dialysis procedures are performed in accordance with clinical protocols and safety guidelines.
• Support staff education regarding dialysis care, equipment, and emergency protocols.
• Participate in quality assurance and performance improvement initiatives.
Communication & Collaboration
• Collaborate with physicians, dietitians, social workers, and case managers to ensure a comprehensive care approach.
• Communicate patient status, concerns, and plan updates to the care team and families as needed.
• Coordinate patient transitions to outpatient dialysis, home dialysis, or other levels of care.
Documentation & Compliance
• Maintain accurate records in the electronic medical record (EMR) system, including assessments, care plans, and treatment summaries.
• Ensure compliance with federal and state regulations (e.g., CMS, ESRD Conditions for Coverage, The Joint Commission).
• Assist with audits, surveys, and internal reviews as needed.
Administrative Support
• Manage dialysis supply ordering, equipment maintenance scheduling, and unit workflow.
• Assist with new patient onboarding and education regarding dialysis modalities.
• Maintain up-to-date protocols and participate in staff training and orientation.
