Overview

This position serves as the point person, coordinator and expert for population health initiatives for the HH Accountable Care Organization (ACO).  Reporting to the Executive Director of the Physician Network, this position works closely with the Director of Primary Care operations and the ACO Medical Director and Medical Director of Primary Care. 

This individual is responsible for creating and operationalizing strategic population health initiatives for the ACO.  They will engage directly with the providers and staff at the practice level, providing guidance and goals for performance in the population health realm.  They will also oversee quality reporting for the ACO attributed patients and oversee the contractual arrangements and compliance for the ACO with CMS.

This individual also provides oversight and performance management of managed care payor patient populations that require population health management, such as Medicare Advantage plans.

Qualifications

Education:HS Diploma or GED required. Bachelor degree in nursing, related clinical field, or healthcare management is preferred.

Experience: Must have a minimum of two years of experience in a population health management setting or other applicable healthcare experience. Must have experience working with varying levels of outpatient healthcare stakeholders, from front line staff to physicians and administrators/executives.  Must be comfortable driving strategic initiatives with little oversight.
